Insight from the Experts

The author, Veronica Walk notes that Clinical Decision Support solutions are gaining traction, yet most are embedded in EHRs and have limited capabilities to address the breadth, variability and complexity of care most patients require. As a result, providers are increasingly seeking AI based CDS solutions with expanded knowledge sets to cover a wider range of clinical demands. As well, providers are seeking the ability to engage their patients in shared decision making with patient-centric CDS.

That’s where the Medical Brain comes in, notes Lauren Arnold, PhD of healthPrecision. The Medical Brain’s patient-centric clinical decision support gathers all patient data and patient generated inputs, analyzes that data to detect health deterioration, care gaps and delays, and errors. Using CDS, the Medical Brain generates clinical insights to the patient so they can take immediate corrective action. Only when needed, providers are contacted for intervention. The result is better disease control, improved patient satisfaction, lower costs and a 92% reduction of provider follow-up tasks.

Patient Centric Clinical Decision Support – An Essential Capability for Providers to Succeed in Value Based Care

Patient Centric Clinical Decision Support – An Essential Capability for Providers to Succeed in Value Based Care

Healthcare Evolution: PCDS and Whole Person Care

In the evolving landscape of healthcare, patient-centric decision support (PCDS) has emerged as a pivotal element, now increasingly intertwined with the concept of whole person care. This holistic approach engages patients in shared decision making and places information and resources in their hands so they can take an active role in driving their healthcare.

PCDS recognizes the multifaceted needs of patients, addressing not only their physical health but also their mental, emotional, and social well-being as well as their preferences and goals for their healthcare. As we transition to value-based care (VBC), the integration of PCDS with whole person care principles is becoming crucial for healthcare providers aiming to optimize patient outcomes, achieve operational efficiency and reduce total cost of care.

PCDS is an advanced amalgamation of data-driven tools and personalized care strategies, emphasizing the unique needs and preferences of patients in the clinical decision-making process. It leverages patient-generated health data (PGHD), electronic health records (EHRs), and analytics to facilitate individualized, evidence-based clinical decisions. In the VBC model, the Centers for Medicare & Medicaid Services (CMS) actively promotes the concept of whole person care and shared decision making as a means of achieving better outcomes and cost structures.

By organizing care around individualized patient requirements and tailoring healthcare strategies to the patient profile, more effective and precise treatment plans can be implemented, leading to increased patient engagement, clinical effectiveness and efficiency.

Patient-centric clinical decision support solutions enable patient centric care. The Medical Brain’s patient centric clinical decision support system is recognized by Gartner as an example of a solution being used by patients and providers for this purpose. By aggregating and analyzing patient data and patient generated inputs, including natural conversations with patients, the Medical Brain captures a complete picture of the patient which includes the history and current situation, constantly updated.

With proactive outreach, the Medical Brain captures patient perspectives on their health status, their symptoms and adherence to care plans, and can detect in real-time health deterioration, care gaps and delays as well as potential errors as well as social concerns. Leveraging evidence-based guidelines the Medical Brain guides patients to take Immediate corrective action, eliminating the normal delays patients experience waiting for providers to respond. It also alleviates provider burden.

With the rapid transition to value-based care, providers are challenged to deliver outcomes while managing total cost of care. The incorporation of patient centric clinical decision support is being shown as a vehicle for success in achieving these goals. With the ever-increasing demands for complex chronic care management by a growing number of patients, providers are adopting technology to help scale their practices to manage more patients and monitor and guide them 24/7. It’s the only way to achieve success with VBC.

Maimonides Medical Center Achieves Groundbreaking Obstetrics Results with the Medical Brain – 91% Reduction in ‘Red Never Events’

Maimonides Medical Center Achieves Groundbreaking Obstetrics Results with the Medical Brain – 91% Reduction in ‘Red Never Events’

Maimonides Medical Center (MMC)

Maimonides Medical Center (MMC), a leader in Maternal Fetal Medicine and the largest obstetrical care provider in New York, and HealthPrecision announce groundbreaking achievements in reducing Obstetrical Never Events by 90.7% with the Medical Brain AI technology. The collaboration between MMC and healthPrecision, led by Maimonides’ Dr. Shoshanna Haberman, MD, Director of Maternal Fetal Medicine and Dr. Eyal Ephrat, CEO healthPrecision places the Medical Brain AI’s clinical decision support and automation in the hands of providers to help those providers deliver high precision obstetrical care. With the Medical Brain, MMC has taken a significant leap forward in combating the rise in maternal morbidity, which has alarmed the nation and highlighted the urgent need to reduce adverse events and improve access for high quality care.

The American Journal of Ob/Gyn Study

The 3-year study of the impact of the Medical Brain’s impact, conducted by the Department of Maternal Fetal Medicine illustrated the Medical Brain’s AI based clinical decision support application had a profound impact on quality, reducing OB red never events from 118 to 11 patients per 1,000. Red never events are those situations that are preventable and place mothers and infants at extreme risk. Red never events include such potentially harmful situations as not discontinuing the administration of Pitocin with a non-reassuring fetal heart rate, lack of administering magnesium sulfate for patients with severe preeclampsia and lack of administering antibiotics to patients with positive group B strep.

“The Medical Brain is effective in helping us reduce never events and improve quality for our high-risk patient population. It is a high-precision, real-time system that accurately sees and constantly analyzes the patient’s clinical picture and identifies any health risks, delays, gaps in care or documentation then guides the clinician so they can take immediate action,” said Shoshana Haberman, MD, PhD, director of maternal-fetal medicine and medical informatics.

A New Era in Obstetrical Care

“Labor and delivery is very dynamic. A lot is happening at the same time in our busy service. So, things can, very quickly, go from normal to problematic for mom and baby,” said Scott Chudnoff, MD, chair of the OBGYN department. “With the Medical Brain, we have a tool that allows us to identify trends or detect when something negative is about to occur. It’s an early detection, warning and guidance system that alerts us to critical events and helps us take timely steps to ensure the safety of our patients.” This advanced approach has not only proven effective in significantly reducing adverse events but also in streamlining care delivery and reducing the burden on healthcare professionals.

Leading the Way in Healthcare Innovation in Obstetrics

“This is the beginning of what the future of medicine will look like, and Maimonides is at the forefront,” Dr. Chudnoff said. “As we integrate and utilize computer assistance more, it will lead to better patient management and less time for patients to spend in the hospital or emergency room.” “The success at Maimonides Medical Center serves as an example for healthcare institutions nationwide, demonstrating the invaluable role of AI in addressing critical healthcare challenges Obstetrics. MMC and healthPrecision are at the forefront of this transformative journey, committed to advancing healthcare to improve outcomes for with technological excellence,” noted Dr. Eyal Ephrat, CEO healthPrecision.

Report of a Research Study Conducted by Maimonides Maternal Fetal Medicine Department – Itamar Futterman, Erum Azhar, and Shoshana Haberman https://www.ajog.org/article/S0002-9378(22)01987-1/fulltext

In recent years, the obstetrics field has been under significant scrutiny due to a disturbing rise in maternal morbidity and mortality rates across the United States. The National Center for Health Statistics (NCHS) and the Centers for Disease Control (CDC) highlighted a nearly 60% increase in maternal mortality rates from 2019 to 2021, alongside an upsurge in severe maternal morbidity. This alarming trend has catalyzed the push towards adopting more robust, evidence-based practices aimed at enhancing maternal health care quality and safety.1

Amid this national healthcare crisis, the Obstetrics Department at Maimonides Health has emerged as a beacon of innovation and hope. Serving a large, diverse, and high-risk patient population, this tertiary care and regional perinatal center faced the common challenges plaguing many busy obstetrical services nationwide: care gaps, delays, and challenges with patient access and accessibility to the right care. The inherent busy nature of these clinical environments often leaves providers stretched too thin, impacting their ability to continuously monitor patients and identify risks in real time.

A Turning Point with AI: The Implementation of Medical Brain

Recognizing the urgent need for change, Maimonides Health took a bold step forward by implementing an AI-based system, HealthPrecision’s Medical Brain, within its Obstetrical Service. This clinical decision support (CDS) platform combines automation and actionable clinical decision support with real-time actionable insights, significantly enhancing providers’ ability to improve outcomes through rapid risk identification and real-time intervention. The Medical Brain aggregates and analyzes patient information from the EMR and other data sources, identifies risks of health deterioration, care gaps, delays and errors, then guides providers in real-time to take corrective action, closing the loop to ensure completion. When needed, the Medical Brain escalates to ensure patients are fully protected.

Impactful Results

The results speak volumes. Following the implementation of the Medical Brain platform, Maimonides Health witnessed a dramatic reduction in the incidence of red never events (RNEs), which are serious and preventable incidents indicating a clear breakdown in the standard of care. The average total RNEs per 1000 live births plummeted from 117.5 to 10.9, marking an astonishing 90.7% reduction. This decline not only showcases the platform’s effectiveness in enhancing patient safety but also illustrates the potential of AI in revolutionizing obstetrical healthcare delivery.

Beyond Numbers: A Change in Care Delivery

The integration of the Medical Brain system into Maimonides’ obstetrical care workflows signifies a transformative leap in healthcare delivery. This sophisticated system offers real-time clinical insights directly to providers’ smartphones, ensuring urgent care needs are met swiftly and in alignment with the latest evidence-based protocols. By reducing the burden of alerts, follow-up tasks, and electronic health record (EHR) documentation by over 90%, it not only enhances patient care but also significantly lightens the workload on healthcare professionals. The Medical Brain stands out for its ability to integrate comprehensive patient data from diverse sources, including EMR, lab results, and radiology, offering a detailed clinical picture that facilitates gap identification in care and documentation. Its proficiency in parsing both structured EMR data and unstructured clinician notes through advanced machine learning and clinical decision support systems is noteworthy. This allows for the early detection of deviations in patient health, offering actionable insights and reducing the incidence of alert fatigue by notifying providers only when necessary, guided by standards from respected entities like ACOG. Furthermore, it bridges the care continuum, benefiting patients in inpatient, ambulatory, and home settings alike, thus encapsulating a holistic approach to patient care and safety.

A Nationwide Model

The success story of Maimonides Health’s Obstetrical Service serves as a powerful model for healthcare institutions nationwide. It underscores the critical role of AI and CDS platforms in addressing the pressing challenges of maternal morbidity and mortality. By harnessing the power of AI technology, healthcare providers can improve clinical decision-making, streamline care delivery workflows, and, most importantly, save lives.

As the healthcare industry continues to evolve, the story of Maimonides Health and the Medical Brain platform offers a glimpse into the future of obstetrical care. It is a testament to the transformative potential of AI in healthcare, paving the way for a safer, more efficient, and patient-centered approach to whole person, high precision maternal care.
